Questions & Answers.
Who are your professional services for?
We work in partnership with NHS Trusts, Integrated Care Boards, Local Authorities, Continuing Healthcare teams, case managers, and other health and social care professionals. Our services are designed to support safe discharges, complex care in the community, and long-term home-based care solutions.
What kind of partnerships do you form with professionals?
We build collaborative, outcome-focused partnerships based on trust, communication, and reliability. Whether it’s short-term step-down care, a complex package, or continuity following discharge, our goal is always to complement your service and provide seamless support for the individual.
How can I refer a client to Ultima Care?
You can refer to us securely via email, phone, or by completing our online referral form. Our professional referrals team will acknowledge receipt promptly, carry out a needs assessment, and keep you updated throughout the process. We accept both contracted and spot referrals.
What documentation or information is required for referral?
We typically require a recent assessment or care plan, relevant risk assessments, and any supporting medical or behavioural information. If these aren’t available, our team can complete the necessary assessments in collaboration with you to ensure safe and timely onboarding.
How do you ensure continuity and communication between professionals and your team?
We maintain clear communication channels through regular updates, joint reviews, and shared documentation. Our team liaises closely with discharge planners, district nurses, and commissioning leads to ensure smooth transitions and consistent care delivery.
What governance and quality assurance frameworks do you use?
We’re regulated by the Care Quality Commission (CQC) and operate an ISO 9001 and ISO 45001 accredited management system. These frameworks guide our approach to quality, safety, and continuous improvement across all services. We also monitor KPIs, outcomes, and client feedback to evidence performance.
Can you support clients with complex or high-acuity needs?
Yes. Our nurse-led model enables us to deliver complex care safely at home — including tracheostomy, PEG, ventilation, spinal injury, palliative, and neurological care. Each package is clinically overseen, with training tailored to the individual’s needs and robust care governance in place.
How do you handle safeguarding and risk management?
All staff receive mandatory safeguarding and risk management training, and we follow strict incident reporting and escalation procedures. Our Registered Manager and clinical leads review all safeguarding activity to ensure compliance with CQC and local authority frameworks.
What geographical areas do you cover for professional referrals?
We currently operate across Kent — including Ashford, Folkestone, Dover, Canterbury, and surrounding areas — with capacity to mobilise complex or interim packages rapidly within these regions.
How can I contact your professional liaison team?
You can reach our professional liaison team directly via phone or email, or by submitting a referral enquiry through our website. We aim to respond the same day, ensuring a clear and efficient pathway from initial contact through to package commencement.
